Respectful Behavior Expected
To ensure a safe and positive experience for everyone, we ask that all patients:
- Treat our staff and other patients with respect.
- Use polite language and remain calm.
- Avoid aggressive, disruptive, or intimidating behavior.
Our team is here to help. If these expectations are not met, we may take appropriate action, including ending enrolment.

How to Enrol:
About Enrolment
Enrolling with our practice means choosing us as your medical centre to provide you with subsidised primary health care services. In New Zealand, if you are eligible for publicly funded health care, you can enrol with one general practice to receive subsidised care.
Eligibility for Publicly Funded Health Services
You must meet eligibility criteria set by the Ministry of Health to enrol and receive publicly funded services. This typically includes residing in New Zealand and meeting the residency requirements. To check if you are eligible, please refer to the Ministry of Health guide to eligibility criteria.
Steps to Enrol
- Complete the Enrolment Form
Carefully fill in the enrolment form with your personal details. Make sure all information is accurate and complete.
- Read Important Documents
Before submitting your enrolment form, please read and understand:
- Terms of Trade & Use Agreement – By enrolling, you acknowledge and accept these terms, including your rights and responsibilities as a patient.
- Use and Confidentiality of Your Health Information (Fact Sheet) – Explains how we collect, use, store, and protect your health information in accordance with the Privacy Act 1993 and the Health Information Privacy Code 1994.
- Complete the Health Questionnaire
Fill out the brief health questionnaire and include it with your enrolment form. This helps us provide the best possible care for you.
- Provide Identification
When you bring your completed enrolment form to the Centre, please provide original copies of your identification documents (proof of identity and proof of address).

Bring original ID documents when you visit to enrol.
Accepted Identification Requirements
You will need:
- Proof of identity, for example:
- New Zealand or Australian passport (other nationalities may need to check eligibility)
- New Zealand birth certificate
- MSD Community Services Card
- Proof of address, for example:
- A rental agreement
- A letter addressed to you at your current address
- Photo ID, such as:
- Drivers licence
- 18+ Card
- Student ID card
Please bring the ID documents listed above with your enrolment form when you come to the Centre.
