Our enrolments are currently open.
To enrol, you must meet the eligibility criteria set by Health New Zealand | Te Whatu Ora. Generally, you are eligible if you are a New Zealand Citizen, Permanent Resident, or hold a visa valid for at least two years.

Not eligible? You may still be able to visit us as a casual patient, depending on appointment availability.
How to Enrol
You must complete a separate enrolment for every individual. Once submitted, we will process your application and request your medical records from your previous practice.
Option 1: Online (Fastest)
Please read the New Patient Information below carefully before starting. Ensure you have your ID documents ready to upload. Click here to complete online enrolment forms
Option 2: In-Person
Visit our reception team at the Centre to complete the required paperwork and provide your original ID documents in person.
New Patient Information
Once your forms are submitted, please follow these steps:

Await Confirmation: We will contact you via email once processed (usually within one week).

Book Your First Appointment: Once confirmed, you can book a consultation with your GP or Nurse Practitioner to discuss your medical history and health concerns.

Join the ‘Well’ Patient Portal: For repeat scripts and results, we recommend joining the Well portal. See our patient portal page for details.

Respectful Behavior Expected
To ensure a safe and positive experience for everyone, we ask that all patients:
- Treat our staff and other patients with respect.
- Use polite language and remain calm.
- Avoid aggressive, disruptive, or intimidating behavior.
Our team is here to help. If these expectations are not met, we may take appropriate action, including ending enrolment.
